Hi all,

First of all, thank you to volunteers for a wonderful retreat last weekend!  I wanted to make sure everyone had the necessary information for our next meeting, March 30, 2008 from 2-4:30pm at Beth Israel's Kirstein Living room.

Parents, you are invited to join us at 3:00pm for Health and Safety Presentations by volunteers and Q&A on Health and Safety with board member/Health guru Nicole Dettman

Parents and Volunteers, please complete the attached Medical Update form and bring it to the meeting.  Please note that the volunteer has two options: "Yes, there have been changes"... or "No, there have been no changes:  "Yes, there have been changes"... or "No, there have been no changes" Whichever one is checked, the vol's signature and that of at least one parent or legal guardian (unless over 18 now), is required. The form will not be accepted without both.  Please read the following instructions.

• Read and fill in the form completely down through the "Yes or No" option.
• If you check the "YES" box, continue reading down and fill in the question below which outlines the change or update in medical or mental heath status.
• Read the last two paragraphs and sign
• Place the form in a sealed white envelope with the vol's name and chapter written on the outside.
• If you check the "NO" box , proceed to the end of the form.
• Read the last two paragraphs and sign the form.
• Place the form in an envelope – you do not need to seal it (unless you are mailing it) as there will be no confidential health information on the form.
• Following completion, you have two options for returning the envelope:

1)      Bring the form to the Amigos meeting on March 30.

2)      Mail the form to Rick Atkins, 76 Middle St., Lexington, MA  02421

Please remember, you are obligated to inform AMIGOS directly if there are any further changes in the vol's medical condition between completion of this form and departure for country.

Volunteers, please remember to complete the online Health and Safety assessments prior to the meeting.  Instructions are below:
Log in and access the online health and safety assessment directly at: http://community.amigoslink.org/forums/thread/1733.aspx  You will be prompted for your login name and password in order to access the online training center.  Or you can go to www.amigoslink.org

1. Click on "Training Center"
2. Then click on "Sign In"
3. Then click under Chapter Volunteer, heading click on "online assessments"
4. Then at the prompt, enter login name and password.
5. This will pull up the online assessment page, click on "Health & Safety Assessment" & read instructions
6. Follow instructions and click on "Link to Health & Safety Assessment"
7. Take assessment. =)
